Joining your virtual visit should be simple.

Back to Life Mental Health uses a secure Tebra Telehealth room for virtual visits. At your scheduled appointment time, use the direct button below. Your Tebra reminder may also include the same visit link.

01Open the secure room

Use the Join Telehealth Visit button on this page or the link in your Tebra reminder.

02Enter your name

Follow Tebra's on-screen prompts to identify yourself for the scheduled visit.

03Allow camera + microphone

Follow the browser prompts and check that you can see and hear clearly.

04Enter the waiting room

Follow the on-screen instructions and wait for the provider to begin the visit.

Before your next visit

Forms and appointment changes, without the guesswork.

Tebra can send patient-specific forms and appointment-management links by email or text. Those links belong to the individual patient and should not be replaced by a generic public form.

Complete requested forms

When Back to Life Mental Health sends intake or update forms through Tebra, open the secure invitation from your email or text message and complete the requested information online.

  • Demographics and emergency contact information
  • Medication, allergy, and medical-history updates
  • Consent or custom practice forms
  • Screening questionnaires when requested

If you expected forms but did not receive them, call the office rather than sending clinical information through ordinary email.

Reschedule or cancel when available

If online appointment management is enabled for your visit, your Tebra email or SMS reminder can contain a link to reschedule or cancel.

  • Open the most recent appointment reminder.
  • Choose the manage, reschedule, or cancellation option shown.
  • A reschedule request may still require confirmation by Back to Life Mental Health.

Keep visit information handy

Before medication-management visits, have important changes ready so appointment time can focus on your care.

  • Current medications and supplements
  • Side effects or meaningful changes since the last visit
  • Requested blood pressure, heart rate, weight, or labs
  • Questions you want to make sure are addressed
Secure messaging

How to message your provider through Tebra.

Secure messages stay within the Patient Portal instead of traveling through standard email or text messaging.

01 / Sign in

Open your portal

Go to the Tebra Patient Portal and sign in with your account email and password.

02 / Messages

Open Messages

From the portal dashboard, choose View Messages to see your inbox and sent messages.

03 / Compose

Choose New Message

Select the provider, add a subject, and write your message. Tebra also allows an image or document attachment up to 10 MB.

04 / Send

Send Secure Message

Review the message and choose Send Secure Message. It is delivered to the provider's Tebra Message Center.

Portal login help

Don't know your login or can't remember your password?

Tebra uses your email address as the Patient Portal login—there is no separate patient username. Back to Life Mental Health can help identify the portal-access problem, but the practice cannot see or retrieve your password.

Don't know your username?

Your login is your email address.

The email address associated with your Tebra Patient Portal account is your login. If you are unsure which email was used, call Back to Life Mental Health.

  1. Call the office at 480-313-8583.
  2. After identity verification, the office can review your Patient Portal status and the email associated with your record.
  3. If an already-activated portal needs its login email changed, additional Tebra assistance may be required.

Forgot your password?

Reset it directly through Tebra.

  1. Open the Tebra Patient Portal.
  2. Select Forgot your password?
  3. Enter the email address associated with your portal account and choose Reset password.
  4. Open Tebra's reset email and use the temporary password provided.
  5. Choose Create New Password Now, then set a new password for future sign-ins.

Password resets require access to the email inbox associated with the portal account.

Don't have an account?

We can send or resend your invitation.

Patient Portal access begins with an invitation from Back to Life Mental Health. The invitation contains a Get Started link.

  1. Open the Tebra invitation email and choose Get Started.
  2. Your invited email address is filled in automatically.
  3. Create your own password and enter your date of birth to activate the account.

If you never received the invitation, it expired, or you are not sure whether your account was activated, call the office and we can review the portal status and resend the invitation when appropriate.

Inside your portal

Records, receipts, payments, and authorized access.

Available features depend on the practice's Tebra settings, but the Patient Portal can bring several common administrative tasks into one secure place.

Review or share your record

Use Health Records to review available medications, problems, allergies, vitals, and lab information. Tebra also provides Actions for downloading a summary of care or sending it to another clinician.

Find shared documents

Use Documents to review items shared through Tebra, such as treatment plans, patient education, or uploaded documents that are available to your account.

Billing

Pay a bill or get a receipt

If payments and itemized receipts are enabled for your account, the portal can show balances, bill details, previous portal payments, and downloadable itemized receipts.

Parents + family

Use authorized guest access

Tebra can let a patient invite an authorized guest to permitted parts of the account without sharing the patient's password. This can be useful for parents, caregivers, or other trusted family support.

For adolescent patients approaching adulthood, portal ownership and family access may need to be updated as the patient turns 18.

Use the right channel

Keep clinical information in the secure portal.

For medication questions, treatment information, or sensitive documents, use Tebra secure messaging. Ordinary email and standard text messaging should not be used to send protected health information. For scheduling questions, call the office or use online scheduling.

The Patient Portal and routine telehealth links are not emergency services. If you are in immediate danger or have an emergency, call 911 or go to the nearest emergency department.
