Tebra can send patient-specific forms and appointment-management links by email or text. Those links belong to the individual patient and should not be replaced by a generic public form.
When Back to Life Mental Health sends intake or update forms through Tebra, open the secure invitation from your email or text message and complete the requested information online.
If you expected forms but did not receive them, call the office rather than sending clinical information through ordinary email.
If online appointment management is enabled for your visit, your Tebra email or SMS reminder can contain a link to reschedule or cancel.
Before medication-management visits, have important changes ready so appointment time can focus on your care.
Secure messages stay within the Patient Portal instead of traveling through standard email or text messaging.
Go to the Tebra Patient Portal and sign in with your account email and password.
From the portal dashboard, choose View Messages to see your inbox and sent messages.
Select the provider, add a subject, and write your message. Tebra also allows an image or document attachment up to 10 MB.
Review the message and choose Send Secure Message. It is delivered to the provider's Tebra Message Center.
Tebra uses your email address as the Patient Portal login—there is no separate patient username. Back to Life Mental Health can help identify the portal-access problem, but the practice cannot see or retrieve your password.
The email address associated with your Tebra Patient Portal account is your login. If you are unsure which email was used, call Back to Life Mental Health.
Password resets require access to the email inbox associated with the portal account.
Reset Through TebraPatient Portal access begins with an invitation from Back to Life Mental Health. The invitation contains a Get Started link.
If you never received the invitation, it expired, or you are not sure whether your account was activated, call the office and we can review the portal status and resend the invitation when appropriate.
Call 480-313-8583Available features depend on the practice's Tebra settings, but the Patient Portal can bring several common administrative tasks into one secure place.
Use Health Records to review available medications, problems, allergies, vitals, and lab information. Tebra also provides Actions for downloading a summary of care or sending it to another clinician.
Open Health RecordsUse Documents to review items shared through Tebra, such as treatment plans, patient education, or uploaded documents that are available to your account.
Open Patient PortalIf payments and itemized receipts are enabled for your account, the portal can show balances, bill details, previous portal payments, and downloadable itemized receipts.
Open Billing ToolsTebra can let a patient invite an authorized guest to permitted parts of the account without sharing the patient's password. This can be useful for parents, caregivers, or other trusted family support.
For adolescent patients approaching adulthood, portal ownership and family access may need to be updated as the patient turns 18.
For medication questions, treatment information, or sensitive documents, use Tebra secure messaging. Ordinary email and standard text messaging should not be used to send protected health information. For scheduling questions, call the office or use online scheduling.
The Patient Portal and routine telehealth links are not emergency services. If you are in immediate danger or have an emergency, call 911 or go to the nearest emergency department.
