01 / Evaluation

Psychiatric Evaluation

A comprehensive first visit to understand symptoms, history, current stressors, medical factors, previous treatment, and what you want help with.

  • Review of current symptoms and functioning
  • Psychiatric and relevant medical history
  • Medication and treatment history
  • Diagnostic assessment
  • Collaborative treatment planning
02 / Ongoing care

Medication Management

Medication is one tool within a larger treatment plan. Follow-up care focuses on response, side effects, function, safety, and whether the current plan still makes sense.

  • Medication response and side-effect monitoring
  • Shared decisions about changes or alternatives
  • Review of sleep, energy, anxiety, mood, and daily function
  • Coordination with therapists and other clinicians when appropriate
  • Education so you understand the purpose of the treatment plan

03 / Follow-up

Progress Visits

Follow-up appointments are used to reassess what has changed since the last visit and make the smallest useful adjustment when one is needed.

  • Symptom and functional progress
  • Medication adherence and tolerability
  • New stressors, medical changes, or other medications
  • Questions and treatment preferences
04 / Access

In-Person + Telehealth

Meet in person at the Anthem office or use telehealth when appropriate. Telehealth is available for patients located in Arizona at the time of the appointment.

  • Anthem office appointments by appointment only
  • Statewide Arizona telehealth access
  • Care for adolescents and adults

How telehealth visits work

Conditions we treat

Mental health concerns can overlap.

Symptoms do not always fit neatly into one category. Evaluation looks at patterns across mood, anxiety, attention, trauma, sleep, medical factors, substance use, and day-to-day functioning.

Insurance & payment

Commercial insurance + private pay

Back to Life Mental Health is in network with select commercial plans. Coverage can vary by specific plan, so behavioral health benefits should be confirmed before the first visit.

Medicare and Medicaid/AHCCCS are not accepted at this time. Private-pay options are available; call for current rates.
